‘Birth Pool In A Box’
Rental Agreement.
The following rental agreement will take place between Durham Doula and the individual named below.
The Birth Pool:
will be used according to the instructions included.
being rented is only meant to be used by the person whose name appears on this form and is non transferable.
will be inspected before you receive it and will be supplied in good working condition, ready for use and will be returned in the same condition received.
must be used with a disposable liner which is included with the rental.

This Rental Includes:
use of professional grade Birth Pool In A Box, regular size
step-by-step setup/takedown instructions
a new fitted disposable pool liner
2 x 25’ drinking water hose
one for clean water only (filling the pool)
one for dirty water only (emptying the pool)
sink faucet adaptors
an electric air pump
a submersible water pump
debris net
rental period is from 37 weeks gestation up until 2-3 days after birth (unless otherwise agreed upon with the owner.)
All pieces included in this rental must be returned, excluding the single use items listed below:
fitted disposable pool liner
debris net
Wet equipment must be thoroughly dried before packing. If the pump is not dried, you may double bag the item to prevent damaging other equipment.
In the event that the birth pool, any part thereof or the additional rental equipment or any part thereof is lost, stolen or damaged in any way or not returned in the same working condition, the renter will be charged the replacement value of the pool or equipment in question. A credit card number is required to be on file, and will be charged automatically if the pool or equipment is not returned in the same working condition.
If the Birth Pool and additional equipment is NOT returned for any reason the full replacement charge of $750 will apply.
It is the renter’s responsibility to familiarize oneself with the Birth Pool In A Box, the additional equipment and to read the instructions included with the rental. In the event that the renter discovers any issue whatsoever, the owner must be notified immediately. Failing to do so makes it impossible for the owner to take corrective action.
Wet or damp items or equipment must be dried before packing if possible or double bagged to prevent spillage (submersible pump/ hoses). Please bag these items separately.
Rental Fees:
Birth Pool In A Box and equipment rental is $20 + HST for the rental period beginning at 37 weeks gestation up until a maximum of 3 days after the intended renter has given birth. This price includes delivery and pick up in Durham Region.
If you require delivery outside of Durham Region, Markham, Scarborough or Stouffville, a $25 delivery / pick-up fee will apply. You may also arrange to pick it up and drop it off yourself for free.
50% of the rental fee will need to be paid upon reserving the unit
this is a non-refundable deposit
the remaining 50% of the fee must be paid before the pool is released to the renter
You will be sent an invoice via email after the signing of this contract. Failure to pay the above fees will result in cancellation of services.
Cancellation and Refunds:
Refunds will not be made after the unit has been received by the renter or for non-use of the pool.
You may cancel your rental any time before the pool is dropped off, however the deposit amount will not be refunded.
Warnings and Precautions:
Damage may occur to the pool, equipment or personal belongings if the included instructions are not followed properly.
Never leave children unattended in the vicinity of the pool, especially if the pool has been filled.
Keep pets and all sharp objects away from the pool, equipment and liner at all times.
Keep all electrical devices a minimum of 3 m or 10' away from the filled pool at all times.
If you or your birth partner suffer from heart disease, high or low blood pressure, do not enter the pool without medical supervision.
The water must be maintained between a temperature of 36-37°C for safe use and delivery.
DO NOT OVERFILL. Fill the pool to just below the indicated fill line and ensure the water never passes this line when people are in the pool.

WAIVER OF LIABILITY:
I, the renter, WILL NOT hold Durham Doula/Sarah Leitch responsible or liable for any injury or damage sustained as a result of misusing the Birth Pool In A Box and included rental equipment.
This agreement is meant to be fair to both parties, so that all parties may feel secure in their mutual commitment. By signing this contract, you agree to pay for services as described above. In return, the doula commits to provide these services. It is understood that neither Sarah Leitch / Durham Doula or my representatives are to be held responsible or liable for any undesirable or unexpected outcome resulting from this unit. Should a disagreement occur, the undersigned agree to resolve issues amicably therefore excluding Sarah Leitch / Durham Doula and its representatives from any penalty or fault. The signatures below indicate both doula and client understand the content and terms herein.
